Your story about your office reminded me of a time I was negotiating for a raise. After some back and forth, my boss said, “God, you’re a pain in the ass.” I agreed. And I got what I wanted.
I did some negotiating/conflict resolution training early on in my career, and the best take-away was "figure out the worst and best that could happen." Once you've identified the worst (and avoided the anxiety death-spiral), figured out where you'll settle from the 100% all good things best options, the pressure is off!
It doesn't hurt to ask, ever.

Genuinely didn't think it was an option to not take the first appointment offered. You mean I don't have to re-schedule my entire week so as not to appear ungrateful that the hygienist is willing to see me??
